Froh Community Home - Admissions


Froh Community Home provides skilled nursing care and rehabilitation to the elderly population. A person can qualify for admissions to Froh Community Home is various ways which will be explained in short and further inquiries contact

We will make an initial assessment visit to your home, hospital or from another facility to assure you that we are able to provide you with the care and treatment that you deserve. If we do not feel we can meet those needs we will refer you to other facilities that may be better to meet your specific needs. We will require that the physician provides us with your medical records and other information that will help us to establish our ability to care and treat you and this must be reviewed prior to accepting you. We again do this because your care and treatment is very important and we want to do the very best for you. If you are being admitted from home and can afford to privately pay (or insurance) for your stay without government subsidy (Medicaid) and:

If your physician recommends you should go to a nursing home (skilled care) and submits the necessary paper work.
If you can no longer be cared for or care for yourself and the physician submits the necessary paper work.
A visit will be provided to assess our ability to meet your needs as we will not admit you unless we can provide the appropriate care and treatment.

If you are being admitted and have government subsidy (Medicaid) and:

You must meet certain criteria that is established by the state to be admitted to a nursing home. This is contingent upon your inability to care for yourself and need the continual care.
If your physician recommends you should go to a nursing home (skilled care) and submits the necessary paper work.
If you can no longer be cared for or care for yourself and the physician submits the necessary paper work.
A visit will be provided to assess our ability to meet your needs as we will not admit you unless we can provide the appropriate care and treatment.

If you are admitted in the hospital for three days (three midnights) and require skilled nursing and/or rehabilitation and have Medicare and:

A visit will be provided by us at the hospital to assess our ability to meet your needs as we will not admit you unless we can provide the appropriate care and treatment.
A secondary pay source to cover your stay if you no longer qualify for Medicare reimbursement
If your physician recommends you should go to a nursing home (skilled care) and submits the necessary paper work.

Other Information

Residents are expected to eat in the dining room and may select from buffet items or order specialty items from the menu.
Each day the resident will be assisted as needed to be dressed in street clothes.
We operate an "Open Door" policy and welcome any information to help your loved one's stay with to be as pleasant as possible.
Our Medical Director visits at least 2-3 times weekly.
