The Villa - Guidelines for Residency


I. GENERAL
  A. Must be 60 years of age or older. A birth certificate, drivers license or other document attesting to age may be required.
  B. Must designate an authorized representative to act on behalf of resident.

II. MENTAL CONDITION/SOCIAL BEHAVIOR

  A. Must be willing to follow policies to maintain residency in the Villa and follow simple directions.
  B. Must be capable of social behavior compatible with congregate living.
  C. Must not have behaviors that are inappropriate for congregate living or that is distracting to residents, visitors, or personnel.
  D. Must not have behaviors or conditions that pose a risk of serious harm, safety, or well-being to self, residents, visitors, or personnel, including destruction of property.

III. MEDICAL CONDITION

  A. Must have medical history and a physical completed within 90 days prior to move in date.
  B. Must have a chest x-ray or tuberculosis intradermal skin test within 12 months prior to move in date.
  C. Must not require nursing care or nursing supervision. Certain services may by provided by Home Health.
  D. Must not require a diet other than that provided by the facility.

IV. MANAGEMENT OF MEDICATIONS
  A. Must be able to safely manage own medication unless otherwise indicated. If assistance is needed, must agree to comply with facility policy for medication assistance.

V. PHYSICAL CONDITION / MOBILITY / TOILETING
  A. Must be able to go to the dining for all three meals.
  B. Must be able to transfer self and be independent in mobility for apartment distances. Assistance can be provided to the dining area and lounge.
  C. Must be independent of toileting needs. Use of bedside commodes, urinals or bed pans are not permitted.
  D. Must have control of bladder and bowel or be able to manage incontinence independently with incontinence products.

VI. PERSONAL HYGIENE / BATHING
  A. Must take a bath or shower once a week. Assistance provided as needed.
  B. Must independently take care of daily personal hygiene in such a way as ensure a reasonable amount of cleanliness.
  C. Assistance with dressing can be provided according to resident's physical limitations. Must keep clothing reasonably clean.

VII. ASSESSMENTS / EVALUATIONS
  A. Done yearly unless there is a change in medical, mental, or physical condition that warrants further evaluation.
  B. The resident's Authorized Representative will be notified as soon as possible of any change that may hinder the resident's ability to maintain residency in the Villa.

VIII. SERVICES NOT AVAILABLE

  A. Management of chronic bowel or bladder incontinence.
  B. Urinary catheterization or care.
  C. Colostomy care, unless done personally by the resident.
  D. Use of bedpans, urinals, or bedside commodes.
  E. Bed baths.
  F. Use of physical restraints.
  G. Surgical dressings other than for routine first aid.
  H. Meal tray service in resident's room greater than one week.
  I. Diabetic residents must arrange for professional foot care if needed.
